Vitamin D and vitamin K are sometimes marketed as though they form an inseparable nutritional duo. The biology is more nuanced.
Vitamin D has a well-established role in calcium metabolism. It helps the intestine absorb calcium and contributes to maintaining the calcium and phosphate concentrations required for normal bone mineralisation. It also has roles in muscle, nerve and immune function.
Vitamin K performs a different job. It is required for the activation—or carboxylation—of several proteins. One is osteocalcin, a protein produced by bone-forming cells. Vitamin K occurs in several forms; supplements commonly contain K1 or K2 forms such as MK-4 and MK-7.
Why are D3 and K2 paired?
This explains the logic behind combining D3 and K2: vitamin D supports calcium availability, while vitamin K participates in activating proteins involved in bone metabolism.
But does taking them together necessarily produce better outcomes?
A meta-analysis of eight randomised controlled trials involving 971 participants reported improvement in total bone mineral density and reductions in undercarboxylated osteocalcin when vitamin D and vitamin K were combined. However, the trials differed considerably, and this does not establish that everybody needs a combined supplement.
More recent research into K2 in postmenopausal osteoporosis has also found improvements in several biochemical markers of bone turnover, while noting that the clinical significance of some changes remains uncertain.
That distinction is important. Supplements should address nutritional needs rather than create the impression that more is automatically better.
